Basic Anesthesia Settings in ICU and Operating Theatres
Basic anesthesia settings refer to the essential monitoring, equipment configuration, and safety parameters required to safely administer anesthesia and manage critically ill patients. Although the ICU (Intensive Care Unit) and the Operating Theatre (OT) have different purposes, both require standardized setups to ensure patient stability, safety, and effective response to emergencies.
1. Key Components of Anesthesia Settings
a. Airway & Ventilation Equipment
Ventilator or anesthesia machine capable of volume and pressure-controlled modes
Oxygen supply, air supply, and backup cylinders
Bag-valve mask (BVM) for manual ventilation
Suction machine for airway clearance
Airway tools: laryngoscopes, endotracheal tubes, supraglottic devices, stylets, bougies
2. Patient Monitoring (Minimum Required Standards)
In both ICU and OT, the essential monitors include:
ECG – continuous heart rhythm monitoring
Pulse oximetry (SpO₂) – oxygen saturation
Non-invasive blood pressure (NIBP) or invasive arterial BP
Capnography (ETCO₂) – mandatory in OT, commonly used in ICU for ventilated patients
Temperature monitoring
Airway pressure and tidal volume (for ventilated patients)
Additional OT Monitoring
Agent analyzer (measures concentration of anesthetic gases)
Neuromuscular monitoring (train-of-four)
Minimum alveolar concentration (MAC) display for inhalational agents
3. Medication & Infusion Setup
Induction agents: propofol, ketamine, etomidate
Analgesics: opioids (fentanyl, morphine), non-opioid adjuncts
Muscle relaxants: rocuronium, vecuronium, atracurium
Vasopressors: noradrenaline, phenylephrine, ephedrine
Emergency drugs: atropine, adrenaline, amiodarone
Continuous infusion pumps for sedation (ICU) or anesthesia maintenance (OT)
