Having one focus for each session to maximise efficiency and progress. After having a focus, we will work together to achieve it through working through examples, having the student work through it alone, assisting where required and then trying more advanced examples to solidify the knowledge. If I feel that a student isn't engaging as much as they could, I switch up the tasks and use maths games (a more visual learning approach), or studying exam questions/mark schemes (if the student has exams ahead), and at any point the student can give suggestions for activities too!
Finally, I end the session with reflection, sometimes verbally to the student if there is time - or if not then I always write up feedback and progress/ things to still work on, and then send that to the student shortly after the session.
