I believe the most important thing is that the student is comfortable. The biggest obstacle for students is confidence and making the lessons relaxing and friendly is very important to me.
If the student wants conversational English lessons, I have some topics prepared like school, work, friends, favourite foods, Harry Potter, etc. and we would talk and if they make a mistake with their speech, I will correct them and explain it.
I would make notes on where I think the student can improve (pronunciation, grammar, etc) and plan future lessons with that in my mind. The conversation can go anywhere but the student should be happy and comfortable.
I may give them a task or homework to practice for the next lesson.
If the student would like to know more about English language theory (aspects like verbs, tenses, grammar etc) with more focus on these areas, I would plan lessons where I would test the student on the different tenses, giving tasks where they would need to use them, etc.
The lesson direction is in the students control. I am there to guide, teach and hopefully create a friendly, positive learning space where my students can feel supported and reach their goals!
