In my experience, the first thing that needs to be worked on is the student's mindset. The student needs to know that Mathematics is not a difficult subject. In fact, it is one of the simplest and most straightforward subjects there is. This is because, it follows the same basic rules, no matter what country you go to, and no matter what question is being answered. Next, I will establish a routine for basic mathematical exams, set by me, and sourced online. These will be administered regularly to the student in order to increase his or her familiarity with such questions, and so that the student can become more competent on an unconscious level, being able to solve complex questions almost without conscious thought. Apart from these regular exams, there will be lessons tailored to the particular need of the student being tutored. Apart from the mathematical lessons, the student also has to learn the mindset required to succeed in mathematics, such as what to do after failure, and how to deal with the pressure of solving multiple questions within a limited period of time. This mindset of a mathematician will be taught by me with the help of some self-development literature and exercises. After everything, rewards will be given such as levels of achievement (Master, GrandMaster, Legendary, etc), as well as other physical incentives like days out and food. Then there will be a feedback process (conducted regularly) where I will get feedback from the student about what works and what doesn't work, and adjust accordingly.
